To add a condition note for a specific condition rating, tap 
for that condition rating and a comment box will appear beneath the condition state rating. Here you can type in or speak (using the dictation feature of your device if available), the notes you want to specify for the condition rating. When done, select again to collapse the device keyboard and hide the comment box. A red dot appears on the message icon when notes are present for a specific condition rating.
You can also enter a note general to the element's condition that is not specific to a any particular condition rating by selecting the Comments text box beneath the condition ratings. Similar to the condition notes, you can type in or speak your notes. When done, select anywhere else on the screen to collapse the keyboard.
